Coordinated Health's nationally distinguished bariatric surgeon, John Dockins, MD, FACS, FASMBS, leads the program's cross-disciplined clinical team and is with you throughout the duration of your weight loss journey. Bariatric Clinical Coordinator, Carissa Hrichak, BSN RN can help you get questions answered, or help referring physicians and healthcare providers refer qualified patients.

Step 01 - Attend an Information Session

Surgical Overview: Interventional/Surgical Option, Indication/Contraindication Review, Review of procedure specific volume

Step 02 - Inititial Consult with Surgeon

Surgical Requirements Review: BMI greater than 40, BMI greater than 35 with a presence of diabetes heart disease, refractory hypertension, clinically significant obstructive sleep apnea

Step 03 - Initial Nutrition Consult

Class #1, Class #2, Class #3, Class #4*, Class #5*, Class #6*

Step 04 - Initial Fitness Consult

Class #1, Class #2, Class #3, Class #4*, Class #5*, Class #6*

Step 05 - Monthly Wellness

Monthly Wellness x 3 or x 6 as directed by insurance.

Step 06 - Clearances

Psychology, Cardiology, Pulmonology, Mammography, Pap Test*, Endoscopy*

Step 07 - Pre-Admission Testing

Labs, EGD, EKG, Chest X-Ray, Sleep Study, Pre-Op Visit/Informed Consent, Pre-Op Nutrition Review

Step 08 - Day of Surgery

1-2 Week Post-Op, 6 Month Post-Op, 1 Year Post-Op, Annual Post-Op(Years 2-6), Post-Op Support Groups, Postoperative Nutritional Counseling, Postoperative Fitness Support
